Treatment Strategy for Edentulous Maxilla with Implant Overdenture

Kiyoshi Koyano

Although the success rate for implant overdentures (IOD) for the edentulous maxilla is reported as lower than for implant-supported fixed prostheses, recent studies have shown better clinical results when 4-6 implants are utilized. However, there is still a lack of clarity about the factors associated with the success of maxillary IOD, such as the number of implants, position of implants, type of attachments, splinted or unsplinted, cleanability, masticatory function, patient satisfaction, complications, long-term survival, etc. Patient factors such as bone quantity and quality, opposing dentition, bruxism, general health condition, and age should also be taken into consideration in treatment planning. This lecture will discuss these factors based on the evidence of clinical outcomes as well as patient-reported outcomes, measures and also research findings on the biomechanics of maxillary IOD.

At the end of the presentation, the participant should be able to

  • recognize the factors accounting for the difference in success rate between mandibular IOD and maxillary IOD
  • recognize the factors affecting the clinical success of maxillary IOD
  • identify the points to be discussed in treatment planning for maxillary IOD
