Introduction to Implant Dentistry

Implant Macrodesign, High Insertion Torque and Immediate Loading

Emile Antoine Chrabieh

Emile Chrabieh relates various features of implant macrodesign - body shape, number and design of threads, and self-tapping threads - to primary stability after placement. He also discusses how self-tapping implants achieve high primary stability, comparing maximum values for insertion torque, Periotest, Osstell ISQ, and push-out testing. Studies on the impact of three different implant macrodesigns and two drilling protocols on primary stability are also presented. Next, this lecture discusses the role of high insertion torque as a prerequisite for immediate loading, and whether it presents a risk in terms of bone resorption. Finally, cases are presented to illustrate how primary stability works in different types of bone, and its effect on the different loading protocols.

At the end of this Congress Lecture you should be able to:

  • explain the impact of implant macrodesign on primary stability
  • describe high insertion torque in regard to immediate loading
  • discuss primary stability in terms of loading protocols
