This lecture is based on more than 25 years of clinical experience with bone augmentation techniques. The two best-documented local bone augmentation techniques are discussed: guided bone regeneration (GBR) and sinus floor elevation (SFE). The GBR technique aims primarily at horizontal bone augmentation, whereas the SFE technique aims at vertical bone augmentation at the floor of the maxillary sinus. Both techniques typically utilize a combination of locally harvested autogenous bone chips and a bone substitute. The biological rationale for this synergistic combination is presented, supported by a series of preclinical and clinical studies. This lecture also stresses the subcrestal position of the microrough implant surface during implant insertion as an important prerequisite for the long-term stability of the implant.
