Prevention of the Biological Risk

This lecture provides evidence to show that biological risk cannot be prevented, only reduced, for dental implants in periodontally compromised patients. Contrary to prior understanding, full-mouth tooth extraction does not result in eradication of all periodontopathogens but rather in their severe reduction. Literature is presented to show that the microflora in periodontally compromised patients does not significantly improve over time, even years after extraction. This lecture shows that implants should be considered only for periodontally compromised patients exercising sufficient oral hygiene measures and receiving adequate supportive periodontal therapy. Additional key factors for stable long-term results are presented, including a good soft tissue seal and free gingival grafts to facilitate oral hygiene procedures.
